You will see a form something like this:

Enter a database name, this name can be anything but should represent the script it is associated with. For test purposes we will use 'mynewdat" as our database names. On the next step you will see a form similiar to this:

Step #2
Enter a username and password for your database, make sure they are secure and don't give out these details to anybody, it could be a security risk. Again the user and pass are upto you but make sure you can remember them and don't confuse them with the database name.

Step #3
Step 3 will have a form similiar to this:

Select the "All Privileges" option(unless you know specifically what this database needs.
The wizard gives us our data so far... we see our
User: datuser
Database: USER_mynewdat
Password **************

Please note "USER" represents your username to your cPanel... so if you login with the user "timdee" that will be the placeholder for your database. So your database would be timdee_mynewdat.
